The GP Surgery Anaphylaxis Kit: What the Guidance Requires

A defibrillator gets the attention, but the emergency kit most likely to be reached for in a GP surgery is smaller, cheaper, and far less discussed: the anaphylaxis kit. Every practice that vaccinates, prescribes, or simply sees patients with known allergies is expected to keep one ready — and Resuscitation Council UK guidance is specific about what "ready" means, down to the gauge of the needle.

Why the Anaphylaxis Kit Is a Different Piece of Kit

Anaphylaxis is a severe, rapid-onset allergic reaction that can follow a vaccination, a new medication, a food exposure, or an insect sting — and it can happen in a consulting room as easily as anywhere else. Unlike a defibrillator, which addresses cardiac arrest, an anaphylaxis kit addresses a specific, fast-moving airway and circulation emergency where the treatment is a drug, not a shock. That means the kit lives on a different footing: it contains a controlled medicine with a shelf life, it must be checked on a schedule, and it has to be positioned somewhere staff can reach it without delay.

What the Kit Should Actually Contain

Resuscitation Council UK guidance sets out a specific contents list for a GP surgery’s anaphylaxis kit, built around intramuscular adrenaline rather than the auto-injector devices patients are given for home use:

  • Adrenaline (epinephrine) ampoules — at least two ampoules at 1mg/mL (1:1000) strength.
  • Needles — four 23G needles, sized for intramuscular injection.
  • Syringes — four graduated 1mL syringes, allowing an exact dose to be drawn up.
  • Oxygen supply — with face masks sized for both children and adults.

The emphasis on ampoules and syringes rather than pre-filled auto-injectors is deliberate. It reflects how the kit is meant to be used by a trained clinician, not a bystander.

Auto-Injectors vs Ampoule and Syringe

Patients who carry their own adrenaline typically have an auto-injector, because it is designed for a layperson to use quickly and safely in an emergency. A GP surgery’s kit works differently, because a clinician drawing up a dose can titrate it precisely — something a fixed-dose auto-injector cannot do for every age and weight.

Where Antihistamines and Steroids Fit

Antihistamines and corticosteroids are not recommended for the initial emergency treatment of anaphylaxis, particularly where airway, breathing, or circulation are affected — adrenaline is the front-line treatment, given first and given promptly. Antihistamines have a role for skin symptoms that aren’t affecting the airway, but they are not a substitute for adrenaline when a reaction is life-threatening, and stocking them as if they were is a misreading of the guidance a CQC inspection is likely to pick up on.

Keeping the Kit Ready: Checks, Storage and Accessibility

An anaphylaxis kit is only useful if its contents are still in date and someone can get to it in seconds. Two practical requirements follow from that:

  1. Regular expiry checks. Guidance recommends anaphylaxis packs be checked to confirm every item is within date, with checks carried out at least weekly — more often where local circumstances call for it. Manufacturer instructions on storage, servicing, and expiry should be followed for every item in the kit, not just the adrenaline.
  2. Immediate accessibility. The kit needs to sit in each location where vaccines are administered, and it should not be kept in a locked cupboard or trolley. A kit that requires a key or a search is a kit that arrives too late.

For a practice fitting out or auditing its consulting rooms, the anaphylaxis kit is a useful test case for the wider equipping exercise: it isn’t enough to buy the right items once. Someone has to own the weekly check, log it, and replace what’s used or expired — the same discipline that keeps every other piece of emergency equipment in a modern GP surgery actually fit for purpose.
